Magnesium antidote2/1/2024 Non-pharmacologic interventions for extravasationįor most medications, the treatment of extravasation is nonpharmacologic in nature however, the efficacy of any specific approach has not been demonstrated in controlled studies. 3-6 Preventive measures include appropriate dilution of medication, infusion of medication via the appropriate rate of administration, ensuring patency of the vascular access device, careful monitoring of infusions during administration, use of clear tape or dressings to allow for visual inspection of the infusion site, and immobilization of the extremity with the IV cannula. The best approach to extravasation injury is prevention. Additionally, administration factors, including the experience of personnel administering the injection, the injection technique, and the number of venipuncture attempts to establish a line, contribute to the risk of extravasation, as does the fragility of the patient’s veins. Cytotoxic agents can be further subdivided into DNA-binding and non–DNA-binding agents. Some drugs, including anti-cancer agents, are directly cytotoxic to cells. Certain drugs cause vasospasms, which result in back pressure at the intravenous (IV) site and may expand the puncture site in the vein, allowing leakage to occur drugs that act as vasoconstrictors can also cause tissue ischemia. Osmolality is also a consideration, as differences in osmotic pressure can damage endothelial cells, leading to potential for drug leakage from vessels. 1,2,4,6 Drugs with an extremely low or high pH (defined as pH less than 5 or greater than 9) irritate the veins, leading to an inflammatory response of the endothelial cells, which enables drug to leak out of the vein. This article summarizes the latest recommendations for treatment of extravasation, and updates a similar article prepared by our group in 2015.Ī variety of risk factors are associated with extravasation: mechanical (cannulation technique and line placement), patient-related (predisposition to infiltration injury, current infection, cognitive or other barriers to communicating pain), and pharmacologic (pH, osmolality, vasoactivity, and cytotoxicity of infusate). For vesicant drugs and chemotherapeutic agents, the incidence has been reported to range from 0.01% to 6%. 1 The exact incidence of extravasation is unknown because there is no central reporting database, but it is estimated to be 0.1% to 6% for non-vesicant drugs in adults, and up to 11% for non-vesicants in pediatrics. Damage from extravasation can progress to a significant degree, causing permanent disability and disfigurement, and necessitating surgical debridement or skin grafting. However, vesicants are differentiated from non-vesicants in that they can cause tissue necrosis, blistering, and ulceration. 2,3 Initial symptoms of extravasation are similar to infiltration and include persistent pain, burning, stinging, swelling, and either blanching or erythema at the site of injection or along the course of the vein. 1 Infiltration, often used in reference to extravasation, refers to leakage of a non-vesicant drug or solution. Extravasation is defined as the leakage or inadvertent administration of a vesicant drug or solution from a vein into the extravascular space.

Youtube playall subscriptions2/1/2024 “I do hope this will go down well with our French and German friends and won't lead to protests at the Commission. “And I hope that this means that train drivers in the UK will be able to play their part in improving rail links from the UK to the Continent.” 'World's universal language'įormer Labour MP Tom Harris, who sits on the board of the UK’s HS2 project added: “It's a great compliment to the UK that English is being considered by the EU as the world's universal language. Officials in Brussels insisted that English would remain as an official language post-Brexit, however, as it has been since 1973 because other member states commonly use English despite nominating a different official EU language.įormer rail minister Paul Maynard, the Conservative MP for Blackpool North, said: “I very much welcome the recognition from the EU that English is the global language. “The Irish have notified Gaelic, and the Maltese have notified Maltese, so you have only the UK notifying English.” “We have a regulation … where every EU country has the right to notify one official language,” she said. Polish MEP Danuta Hübner, chairman of the European Parliament Constitutional Affairs Committee, said in 2016 that English should not be recognised. The decision is likely to rankle MEPs who called for English to be banished as a language in the wake of the UK’s Brexit referendum. John Penrose, Conservative MP for Weston-super-Mare, said: “Who'd have thought our influence in the EU would be bigger now we've left? It looks as though someone in Brussels has been missing the British sense of humour.” Controversial decisionĮnglish is being considered as the common language for drivers when driving to neighbouring countries, sources added. They warned that the decision could yet be delayed, however. Officials have instead sought to standardise the language used by train drivers across all member states.īrussels sources confirmed that an update to the Train Drivers’ Directive is planned for autumn this year. The EU has been pushing for “a single European railway area” for a number of years to facilitate easier movement on the railways across the bloc.īut rules stipulate that train drivers must possess a minimum language proficiency in every country they operate. Sources say that English is set to be selected, despite misgivings among some EU member states following Brexit. Rules to be announced this autumn under the EU’s "Train Drivers' Directive" will mandate that a single common language is enforced. You have to be very careful-you have to actively manage subscriptions to apps.Brussels is to force European Union train drivers to speak English under rules designed to foster closer ties between member states. “And it’s effective, because even I sometimes wonder, why am I getting charged this much by Apple every month? And it's like, OK, there’s the shared family storage, there's AppleCare for my phone, there’s Duolingo. “We define fleeceware as something that charges an extraordinary amount of money for a feature that is available freely or at very low cost elsewhere,” he says. Gallagher says that one of the biggest problems with fleeceware is that users don't always know how to manage their subscriptions and don't realize that even when they delete an app, their recurring payments will continue to be active with the service. And instead of limiting the user to a small number of queries, some of the apps would truncate responses and give users only a snippet until they started a subscription. The researchers say they suspect that some of the apps use OpenAI’s ChatGPT 3 application programming interface to generate content for users while others use lower-quality chatbot functionalities. The researchers first learned about the scam apps after seeing ads for them in news apps and on social networks, but users may also encounter them by searching in Google Play and the App Store. The scam apps take advantage of people who have heard about this new technology-and perhaps the frenzy of people clamoring to use it-but don’t have much additional context for how to try it themselves. There are paid versions of OpenAI’s GPT and ChatGPT for regular users and developers, but anyone can try the AI chatbot for free on the company’s website. Today, researchers from the security firm Sophos are warning that the latest incarnation of this is showing up in Google Play and Apple’s App Store, where scammy apps are pretending to offer access to OpenAI’s chatbot service ChatGPT through free trials that eventually start charging subscription fees. In recent months, it has become clear that the same would happen for large language models and generative AI. Any major trend or world event, from the coronavirus pandemic to the cryptocurrency frenzy, will quickly be used as fodder in digital phishing attacks and other online scams.
